Report an incident while it’s fresh, even a near miss. Everything about it stays in one record: what happened, the photos, what was done straight away, the investigation and, where the law asks for it, the statutory facts.
Report an incident
On the phone: open the project’s Site → Incidents and tap Report an incident, or take a photo and choose Report incident. On the web: Site → Incidents → New.
- The incident. The type (Injury, Near miss, Property damage, Environmental, Security or Other), what happened, photos, and the date and time.
- Details. The immediate actions taken, whether it was a lost-time incident and how many days were lost, and where it happened.
The phone can record the coordinates of where you are when you create the report. It only does this at that moment, never in the background.
Follow it through
An incident moves from Open to Investigating, Resolved and Closed. A closed incident can be reopened.
On the web, open the incident and choose edit to add the rest: severity, the people injured, witnesses, the weather, and the investigation (root cause, corrective and preventive actions, who investigated and when, and whether it’s not started, in progress or complete).
UK: RIDDOR
On the web, a UK company’s incidents have a RIDDOR section. Say whether the incident is reportable and choose the category:
- Death
- Specified injury (fracture, amputation, loss of sight …)
- Over-seven-day incapacitation
- Member of the public taken to hospital
- Occupational disease
- Dangerous occurrence
- Gas incident
Konverge works out the latest date the report is due: 10 days after the incident for a death, a specified injury, a member of the public taken to hospital or a dangerous occurrence, and 15 days for an over-seven-day injury. Deaths and specified injuries must also be reported to the HSE straight away, by the quickest means. When you’ve reported it, record the date and the HSE’s reference.
US: OSHA
On the web, a US company’s incidents have an OSHA section: whether the case is recordable, the case type (death, days away from work, job transfer or restriction, other recordable case) and the number of days away and on restricted duty.
Good to know
- Konverge doesn’t report to the HSE or OSHA for you. It keeps the facts and works out the due date; making the report is still your responsibility.
- Reporting an incident needs signal. If you have none, take the photos now (they save on the phone) and report it as soon as you’re back online.
- On the web, Download PDF makes an incident report with your letterhead, for your insurer or the client.
